Program Information
Cubbies: Children in the two years prior to Kindergarten. All Cubbies must be fully potty trained and meet the school age cutoff. A parent or guardian must remain in the building during the first two weeks to help with the transition.
Sparks: Kindergarten through 2nd Grade
Truth & Training (T&T): 3rd through 5th Grade
Trek: 6th through 8th Grade
Journey: 9th through 12th Grade. High school students participate in Bible study and are encouraged to serve as Junior Leaders.


Please complete a separate form for each child you are registering. This helps us ensure that each child's information, grade level, and program materials are recorded accurately.

AWANA Clubber Behavioral Contract 2026/27

The following expectations are for ALL Clubbers who attend the Elgin Evangelical Free Church AWANA Program.  




  • Be respectful of all clubbers, leaders and other individuals in the church.  Listen to others, help each other and be polite.

  • Follow a hands off policy.  Absolutely NO fighting, hitting, punching.  Keep hands and feet to yourself.

  • Use appropriate language.  No swearing, name calling or insults.

  • Be respectful of the church building.  Use all furniture properly and pick up and discard trash.  No running in the hallways, keep the bathrooms clean.  This is God's House!

  • To the best of your ability, work in your handbook.  Try!


Clubbers who cannot follow these expectations will be reminded about their behavior in the following manner ...


  1. Warning and reminder of the rules.

  2. Removal from the situation and discussion with the club director.

  3. Parent contact

Medical Release


Special Events Permission and Medical Release Statement:  9/26-4/27

As a parent/legal guardian, I do authorize for the above named minor, to receive treatment by a qualified and licensed medical doctor in the event of a medical emergency that, in the opinion of the attending medical personnel, may endanger his or her life, cause disfigurement,  physical impairment, or undue discomfort if delayed.  This authority is granted if efforts to contact me, prior to treatment, fail.

The minor has permission to participate in the AWANA Clubs program at the Elgin Evangelical Free Church.  As a parent/legal guardian, I do release the Elgin Evangelical Free Church and its agents, of any liability, in the event of an accident or mishap resulting in the injury of the above named minor.


This release is agreed to of my own free will with the purpose of granting the above named minor permission to participate in and to authorize medical treatment in emergency circumstances, in my absence.

Photography + Filming Notice

Elgin Evangelical Free Church (EEFC) plans to take photographs, audio, and video during our in-person events and reserves the right to use any photograph or video taken at any EEFC event.  By attending, you consent to the use of these materials in church publications and/or on our website.
